Counselling Feeback Form

We understand that during difficult times it can be hard to know who to turn to. Sometimes your thoughts and feelings can leave you and those who are important to you, struggling to cope.

Here at Kaleidoscope, our counselling sessions offer you a safe space to talk about the issues you’re facing and explore difficult feelings in a confidential, non-judgemental environment. We are continuously trying to improve and develop the service so that we can provide crucial mental health support services to the community, wherever they are needed. The most important way for us to do this is to gather feedback from individuals who have been supported by us.

Below is an evaluation form. As someone we have supported, we would love to get your feedback on the service and really value your honest opinions and suggestions for how we could improve. Your comments will be anonymous, however, if you would like us to contact you regarding any of your comments, please include your name, email and telephone number at the end of the form.

Please be assured that if needed, you can always re-refer yourself back to our service, or share our details with anyone else you feel may need our support. We are always here to listen.

Your experiences could bring comfort, courage, and hope to someone who is currently struggling. Many people tell us that hearing real stories from others helps them feel less alone and more confident in reaching out for support. If you feel willing, we’d be grateful for anything you’d like to share about your mental health journey—whether that’s a few written sentences, an audio recording, or a short video. Sharing is completely optional, and you’ll remain in full control of what is used and how it’s shared. If you’re open to participating in a written, audio, or video case study, please leave your details below and our PR & Communications team will get in touch to explain the process and answer any questions. Thank you so much—your voice has the power to help someone take their first step toward support.
